The information below discusses essential documents you will want to have in place as well as some additional documents to make aspects of caregiving less stressful. There are some documents that you can download and fill out from this site and others that you will need to get from a medical or legal professional. Never make assumptions about legal documents. Medication List

This downloadable chart will allow you to record a list of all the medications that the person you are caring for is taking. Print out copies of the medication list and bring it with you to any medical appointments. Make sure you update the form if there are any changes to these medications.
