Requirements
No specific requirements in regards to age. Encourage to bring appropriate documentation such as: filled out application, government issued ID, proof of address/income, and work related documents if applicable. Bring insurance documents if insured. This is not necessary but it is encouraged. Patients are encouraged to call or visit the office for further information about mobile dental clinic locations.
Insurance
Not needed, but will accept insurance. Private insurances accepted, and Medicare/Medicaid.
Cost
Cost will be determined based on individual situation and services. Some patients may receive a discount if eligible.
