Requirements
Individuals must complete the application process through the phone or online. In order to be considered eligible: the disease
being treated for must be part of their list of diseases, medication must be specific to disease, must have insurance that pays
for the medication, there will be income guidelines considered, and individuals must live and have treatment in the U.S.
